High-Flow Nasal Cannula (HFNC): What It Is, Flow, FiO₂ and When to Use It

Of all the ways to give oxygen, high-flow nasal cannula (HFNC) is the one that has grown most these past few years, and you see it just as often in the ICU and the ER as in respiratory care at home. The idea is simple: deliver far more flow than a normal cannula, but with the gas heated and humidified and with a FiO₂ you set separately. The result is more oxygen, more comfortable and better tolerated. Let's look at what it is, how it differs from the ordinary cannula, the flows and FiO₂ it handles, when it's indicated, and what to check when you buy it in volume.

How it differs from the ordinary cannula

A high-flow nasal cannula delivers gas at a flow well above the conventional one, up to about 60 L/min in adults, heated to body temperature and humidified, with a FiO₂ you adjust independently of the flow. The everyday nasal cannula works at 1–6 L/min and its FiO₂ depends on the flow and on how the patient breathes.

The HFNC prongs are wider and softer, and the circuit carries a heated line. For low-flow oxygen therapy the ordinary nasal oxygen cannula and, for sensitive skin, the soft silicone one still do the job.

Why all that flow oxygenates better

The interesting part is that the benefit isn't just "more oxygen": the high flow does several things at once. It flushes the CO₂ left in the nasopharynx, the dead space, so each breath counts for more. It also creates a slight positive pressure in the airway, like a gentle CPAP, which helps keep the alveoli open. And because it delivers a flow that matches or exceeds the patient's peak inspiratory demand, the FiO₂ they actually receive stays stable and doesn't get diluted by room air.

On top of that comes the active humidification: heating and moistening the gas avoids the dryness and mucosal damage that would make such a high flow unbearable if it weren't conditioned. That's why it's far better tolerated than a mask at the same flow, and the patient can talk, eat and cough with it on.

When is it indicated?

It's used in quite a few hypoxaemic respiratory-failure scenarios: pneumonia or another acute hypoxaemia that won't come up with conventional oxygen, the period after an extubation in at-risk patients so they don't relapse, pre-oxygenation before intubating or during a procedure, selected COPD and chronic disease, and support in palliative care. There are also size-specific devices and cannulas for paediatrics and neonatology.

The decision, the starting parameters and the monitoring of the response — saturation, ROX index, work of breathing — are always the clinician's call. And it's worth being clear about one thing: HFNC does not replace mechanical ventilation when that is what's needed.

Choosing the cannula and the size

Four things matter when you pick the cannula and the consumables. The prong size has to leave at least half the nostril clear, because the system needs that leak; that's why neonatal, paediatric and adult (S/M/L) sizes exist. The cannula has to mate with your brand's high-flow device and heated line. The material, latex-free, plus good facial fixation, keeps prolonged use from causing pressure injuries. And, like anything that touches the patient, it's single-use and lot-traceable.

For low or medium flows and for transport, the disposable oxygen mask and the cannula with humidifier bottle are still useful. And for therapy at home, the 10 L oxygen concentrator sorts out the gas source.

Buying in volume: what to look at
